Mission2EndDiabetes Project

This web site was developed as part of the CHRISTUS Health Mission To End Diabetes program throughout Louisiana. 

Providers will maintain a directory of Members with diabetes by using the CHRISTUS Chronic Disease Registry. This registry provide a comprehensive series of reports and useful information about these Members, allowing the provider to intervene to help their Members face the everyday challenges that diabetes presents, leading to positive lifestyle changes and better disease management.

Coordinators and CHRISTUS administrators will also use the CHRISTUS Chronic Disease Registry to perform other administrative functions.

Members will use ConnectPoint to find an assortment of articles, resources, and many fun activities to help them better manage their diabetes.  The premise is simple: learn, have fun, and get healthier!  There is a lot that a Member can do here. A Member can...

• have full control over their content -- and who sees it. They can personalize their privacy settings, text messaging options, and more.

• add friends and family as Supporters to encourage them while they exercise and make healthy lifestyle changes -- they can even let their Supporters challenge them to do fun activities that will help improve their health!

• check out some recipes specific to diabetes to find tasty meals and treats anyone, diabetic or not, will enjoy!

• earn reward points by reading articles and taking part in challenges with their Supporters, and redeem these points for various goodies.

• keep track of their progress by checking their statistics -- helping them adjust their activities accordingly. 

• set goals, and check out the Important Dates calendar to see what they have coming up, or what they may have forgotten.

Coordinators will use ConnectPoint to maintain the general upkeep of system data files and processes for their region.  They will also use the site to add articles and other informative resources for Members in their region, and beyond, including important announcements.  Coordinators  will also use the site to create and view a variety of reports, each outlining the status of the Members within their region.

Supporters will use ConnectPoint to periodically check on Members that they signed up to support.  They will be able to view selected information for a particular Member, including goals, recipes, games, and more.  Supporters can also participate in challenges with a Member (if the Member allows).
