Mission to End Diabetes (M2ED) Applications Privacy Policy

This Mission To End Diabetes (“M2ED”) – USE OFFICIAL NAME- Privacy Policy (the "Policy") governs your use of M2ED’s website located at http://www.mission2enddiabetes.org/ and related websites (collectively, the "Mission to End Diabetes’ sites which are owned by and operated by Button Systems, Inc. and CHRISTUS Health), the M2ED application link on CHRISTUS Health affiliates’, other websites which the link may appear. This Policy does not apply to ideas, concepts, suggestions or inventions that you send to via email to CHRISTUS Health or Button Systems, Inc. Please do not send such correspondence to CHRISTUS Health or Button Systems, Inc. if your intent is to keep such matters private.

The M2ED Site may occasionally provide links to other websites owned and/or operated by third parties. CHRISTUS Health shall not be responsible for the operation of such other sites, including such other site's privacy practices or lack thereof. When visiting any such third party site you should review the privacy statements/policies of each site which you end up visiting through your use of the M2ED Site.

Registration and Access to Services

Although many aspects of the M2ED Site are available without having to register or sign up, some of the content and functionality of the M2ED Site, including the ability to become a member or supporter, require you to register. If you register on the M2ED Site to gain access to other content or functionality on the M2ED Site, you will be required to submit information which could be used to personally identify you ("Personally Identifiable Information"). If you register as a member, information concerning your health including, but not limited to, your weight, height, current and past treatments and medications, symptoms, and other related medical information ("Personal Medical Information") may also become part of the online personal health record. In order for the M2ED Site to be beneficial to you and operate in the manner they were designed to operate, you must provide Personal Medical Information and Personally Identifiable Information that is accurate and complete.

Information We Collect About You

When you register on the M2ED Site, Button Systems, Inc collects Personally Identifiable Information about you. Button Systems, Inc. utilizes this Personally Identifiable Information to provide you with the ability to set your security settings for access and use and communicate with you regarding significant changes to this policy. It is your responsibility to ensure that your Personally Identifiable Information is complete and accurate at all times.

Use of the M2ED Site by Children

CHRISTUS Health believes that protecting the privacy of children is important. Although the content on the M2ED Site and the personal health record site may be used by a parent or guardian of a child to benefit such a child, neither M2ED nor any of its services are designed or intended to attract children under the age of 18.

M2ED’s Disclosure of your Information

Except as set forth in this Policy or as specifically agreed to by you, CHRISTUS Health and Button Systems, Inc. will not disclose any Personally Identifiable or Personal Medical Information gathered from you on the M2ED Site or the personal health record. You agree that CHRISTUS Health may release Personally Identifiable or Personal Medical Information: (1) to comply with valid legal requirements such as a law, regulation, search warrant, subpoena or court order; or (2) in response to a physical threat to you or others, to protect property or defend or assert legal rights. In the event that Button Systems, Inc is legally compelled to disclose your Personally Identifiable or Personal Medical Information to a third party, CHRISTUS Health will attempt to notify you unless doing so would violate the law or court order. In addition, we may disclose Personally Identifiable Information as described below.

M2ED’s programming and service providers may have or be granted from time-to-time limited access to your Personally Identifiable Information in the course of providing services to CHRISTUS Health in support of the M2ED Site and/or personal health record. Such third party providers may include vendors and suppliers that provide CHRISTUS Health with technology, services, and/or content for the operation and maintenance of the M2ED Site and/or personal health record. Access to your Personally Identifiable Information by contractors is limited to the information reasonably necessary for the contractor to perform its limited function for M2ED. CHRISTUS Health requires all third party service providers to execute binding agreement in which the third party agrees to: (1) protect the privacy of your Personally Identifiable Information consistent with this Policy, and (2) not use or disclose your Personally Identifiable Information for any purpose other than providing CHRISTUS Health with products and services as required by law.

CHRISTUS Health may also use your Personally Identifiable Information, along with the Personally Identifiable Information of all or some of the other users of the M2ED Site for research purposes (the “De-identified Aggregated Data"). CHRISTUS Health may desire to use De-identified Aggregated Data in an effort to discover certain trends in testing results, certain aspects of the personal health record which require refinement, etc. CHRISTUS Health may also use the De-identified Aggregated Data to provide third parties with information on user patterns such as the time users spend on a certain portion of the M2ED Site, the average age of individuals participating, etc. CHRISTUS Health requires parties who receive De-identified Aggregated Data to agree that they will not attempt to make this information personally identifiable, such as by combining it with other databases.

M2ED’s Ability to Change the Policy

CHRISTUS Health reserves the right, in its sole and absolute discretion, to amend this Policy from time-to-time. In the event such an amendment constitutes a "material change” (as defined below) in the collection and/or aggregation of Personally Identifiable Information, CHRISTUS Health will notify you in advance of such proposed change and will provide you with the ability to cancel your account on M2ED. For purposes of this Policy, "material change" means a change which expands CHRISTUS Health’s ability to collect, use or disclose Personally Identifiable Information and/or Personal Medical Information. If, following CHRISTUS Health’s adoption of a material change, you continue to use the M2DC Site, such continued use will constitute your acceptance of such material change(s). If you decide that any of the material changes are not acceptable to you then you should immediately stop using the M2ED Site and submit an account cancellation by contacting your Provider Office/Clinic.
